Trinity Home Care

Post-Hospital Home Care in Greenville & Spartanburg, SC

The first two weeks after a hospital stay are when seniors are most likely to fall, miss medications, skip meals and end up back in the emergency room. Post-hospital home care from Trinity Home Care bridges that gap for families in Greenville and Spartanburg: a registered nurse reviews the discharge instructions with you, and a trained caregiver provides the hands-on help that makes recovery at home safe.

Calling from a hospital room? We handle this every week. Call (864) 777-0035 and we can often arrange a nurse visit the same or next day.

What post-hospital care includes

  • Nurse review of discharge paperwork, new prescriptions and follow-up appointments
  • Medication reminders and a reconciled medication list for the family
  • Help with bathing, dressing and mobility while strength returns
  • Meal preparation that follows the discharge diet (low sodium, diabetic, soft)
  • Transportation to follow-up visits, labs and therapy
  • Fall-prevention setup in the home and supervision during the highest-risk days
  • Coordination with home health nurses, physical therapists and your physician

Hospitals and rehab centers we commonly support

Prisma Health Greenville Memorial, Patewood and Greer Memorial; Bon Secours St. Francis Downtown and Eastside; Spartanburg Medical Center, the Mary Black campus and Pelham Medical Center; and skilled nursing and rehab facilities across both counties. Discharge planners are welcome to refer a patient directly.

Recovering from surgery at home

Hip and knee replacements, cardiac procedures and abdominal surgeries all come with the same instruction: do not do too much, and do not fall. Our caregivers make that realistic, from the first careful shower to getting to physical therapy on time. Read our nurses’ guide to hip replacement recovery at home.

Frequently asked questions

How fast can you start after a discharge?

Often within 24 hours. If you call while your loved one is still in the hospital, we can usually have the nurse visit and the first caregiver shift arranged for the day they come home.

Is this the same as home health?

No. Home health is skilled, physician-ordered care (nursing, therapy) billed to Medicare and limited to short visits. We provide the hours of hands-on daily support in between, and we coordinate with the home health team.

How long do families typically keep post-hospital care?

Many start with a few weeks of daily visits or short-term 24-hour care and taper down as strength returns. There is no long-term contract.

Does Medicare pay for this?

Medicare does not pay for non-medical home care. Private pay and long-term care insurance are the usual options, and we help veterans apply for VA Aid and Attendance.
